Voucher Credit Form – For Trade Use Only

Please use this form to submit a credit request for Dengie feed vouchers redeemed in your store. Upon receipt of your claim, we will process and arrange for a credit through the wholesaler you have selected in the form. Please allow two months to receive the credit. We reserve the right to refuse a credit request if we believe it is fraudulent, and therefore ask for you to keep hold of the vouchers until you have received your credit.

Vouchers must be submitted for redemption within one month from the stated expiry date on the front of the voucher.

You can submit a credit for more than one voucher at a time, simply use the +/- functions on the form to add another row.
